How to Apply
OCREB Submissions
- Electronic submissions of initial applications, amendments, annual renewals, study closures and updated investigator brochures are now being accepted at OCREB.Submissions@oicr.on.ca only after the full paper submission is complete and signed.
Instructions
- Download and save the applicable submission form;
- Open the form in MS Word and complete it by typing information into the fields;
- Save the form and email it with the required attachments to: OCREB.Submissions@oicr.on.ca;
- Obtain the appropriate Investigator's signatures;
- Please send the complete submission package by courier to:

Hard copies of all submitted documents plus one additional copy of the signed application form (i.e., one original plus one copy) are required in the paper submission package. For initial provincial submissions only, a total of five paper copies of the protocol and Investigator Brochure (IB) are required if they are not provided electronically. Applicants are encouraged to have the sponsor provide the IB and protocol on CD as part of the submission package. Include only one hard copy of any external safety reports/SAEs issued since the IB was issued (cut-off date) – no electronic copies required.
Send all documents that are available in electronic format to OCREB.Submissions@oicr.on.ca only after the signed paper submission package has been submitted.
Submissions for full board review will be accepted for the next REB meeting if the complete signed package is received at the OCREB office by noon on the submission deadline date.
